I can’t get an appointment for my child’s physical examination until after try-outs begin. What should I do?

If your child has participated on a previous MAU athletic team, you may be granted a waiver until the date of the scheduled physical. Inside the boxed-in portion of the Physical/Participation form you will notice three parts. Under II-B, fill in the date of the scheduled physical appointment for your child and sign it verifying the date. Upon completion of your child’s exam, make sure to get a copy of the exam signed by your physician to your child’s coach so it can be attached to the form on file.

Print out the Physical Participation Form.
